Enrollment forms for the 2012-13 school year may be downloaded via Denver Public Schools’ Office of Choice and Enrollment Services or picked up at the school.
Depending on your child’s age, you must also present copies of some/all of the following when you submit enrollment forms,
- your child’s birth certificate
- immunization records
- proof of your home address (utility bill, copy of lease etc.)
- proof of income
- statement of health
